RCTI Agreement 
By checking the box above and informing Nu Skin Enterprises Australia Inc. (“Recipient”) that I/we (“Supplier”) am/are registered for GST in Australia, I/we agree to the following terms and conditions of this RCTI Agreement entered into between the Recipient and the Supplier: 

1. The Recipient will issue a Recipient Created Tax Invoice in respect of the commissions it paid to the Supplier. 
2. The Supplier will not issue tax invoices in respect of these commissions. 
3. The Supplier is registered for GST purposes in Australia at the time of accepting this agreement and will provide or have provided its ABN to the Recipient. 
4. The Supplier must notify the Recipient immediately if the Supplier ceases to be registered for GST purposes in Australia. 
5. The Recipient will notify the Supplier immediately if the Recipient ceases to be registered for GST purposes in Australia. 

DECLARATION

I declare that:
The information I have provided in this form is complete true and correct.

I acknowledge that:
- Providing false or inaccurate information on this form is a violation of Nu Skin’s Policies and Procedures.
- As a Brand Affiliate, it is my duty to keep the information provided to Nu Skin current and accurate. I must immediately inform Nu Skin of any changes to the status of my GST registration.

By signing this form, I agree that Nu Skin Enterprises Australia, Inc. and its affiliated companies (collectively, “Nu Skin”) may collect, use, process and disclose my personal information given in this form for the purpose of administration in relation to the matters referred to herein. I confirm that I have read and agree to Nu Skin’s Privacy Notice and Section 2, Chapter 1 of the Policies and Procedures and I consent to Nu Skin’s collection, use, processing and disclosure of my personal information in the manner set out therein.
