CONVERSION OF BRAND AFFILIATE ACCOUNT TO MEMBER ACCOUNT

• Please make sure to use the email address that is registered under your account, otherwise we cannot process this request and the account holder will be informed.
• Please check if all provided data is correct before submitting your request.
• By pressing the 'submit' button, we will automatically receive your completed request form. An automatic confirmation email will be sent by the system to acknowledge that the form has been submitted.
• Once your request is processed, our Customer Service team will get back to you via email at your registered email address.

I understand the conversion of a Brand Affiliate Account to a Member Account involves two steps: the first step is termination of an existing Brand Affiliate Account; and the second step is registration of a new Member Account. I understand that I will retain my existing Nu Skin ID after such conversion.

I hereby request to terminate my Brand Affiliate Account and register as a Member, effective as soon as Nu Skin completes the processing of my request.

(a)    Termination of my Brand Affiliate Account results in the loss of all rights and benefits as a Brand Affiliate, including the permanent loss of my Team, and any unpaid accrued bonuses that may be forfeited pursuant to the Policies and Procedures.

(b)    After my Brand Affiliate Account was terminated, I may apply to become a Brand Affiliate again by registering on Nu Skin platform (including agreeing to a Brand Affiliate Agreement).

(c)    I may establish a new Brand Affiliate Account under a new Sponsor only if I have not engaged in any Business Activity (whether for my Brand Affiliate Account or the Brand Affiliate Account of another person) for an inactive period of 6 months or 12 months (if I ever achieved Brand Representative or higher in the 24 months preceding the most recent Business Activity).
