Bank Account Information such as the first page of the bank book, a bank statement or ATM Card from a registered bank in Hong Kong (applicable to Hong Kong brand affiliates) / registered bank in Macau (applicable to Macau brand affiliates) for verification purposes.

BRAND AFFILIATE AGREEMENT AND POLICIES AND PROCEDURES

當您勾選「我同意」並按「提交」您的申請，即表示您確認您有權提交本申請（如屬單獨個人之申請，您必須為申請者本人。如屬商業機構之申請，您必須為該商業機構之被授權代表。如與其他品牌專員共同參與品牌專員帳戶，您必須為主品牌專員），並且您已閱讀並同意接受和遵守本公司的品牌專員協議書和私隱政策之條款及條件，以及本公司的政策與程序。您在此同意如新為處理您的申請相關事宜及為如新政策與程序第一章第二節所述之目的而收集、使用和披露您於本申請中提交之個人資料。您也同意如新得將您於本申請中提交之個人資料轉移至中國大陸、台灣、澳門及香港以外地區。如您的資料有所變更，您會立即通知如新。您明白您可以致函美國如新企業香港分公司於香港銅鑼灣恩平道28號利園二期10樓或致電(852) 2837 7700聯繫本公司的資料保護主任，以要求查閱或更改您所提供之個人資料。您亦同意就此等請求繳交合理之手續費用。請注意，您申報的所有資料必須屬實，否則您的申請將無效。

The Policies & Procedures describe your rights and obligations as an independent brand affiliate. For your convenience, you may also print out the Brand Affiliate Agreement and Policies and Procedures for future reference.

You agree that Nu Skin may collect, use and disclose your personal data given in this application for the purpose of administration in relation to the matters referred to herein and for the purposes set out in Section 2 of Chapter 1 of Nu Skin Policies and Procedures. You also agree that NU SKIN may transfer your personal data in this application to the Mainland China, Taiwan, Macau and anywhere outside of Hong Kong. If there is any change to your personal data, you will notify NU SKIN immediately. You understand that you may contact our Data Protection Officer by post to Nu Skin Enterprises Hong Kong, LLC of 10/F, Lee Garden Two, 28 Yun Ping Road, Causeway Bay, Hong Kong or by telephone (852) 2837 7700 to request access to or correction of the personal data you have provided. You also agree to be charged a reasonable fee for your data access request. Please note that all the information you have provided to us must be true. Otherwise, your application will be void.
