我/我們要求由日期*起終止我/我們的自動訂貨計劃申請合約（「ARO合約」)。
I/ We request to terminate my/our Automatic Re-ordering Program Enrollment Agreement ("ARO Agreement") effective from Date.

我/我們明白及同意若我/我們在合約到期前終止ARO合約，如新可從我/我們品牌專員帳戶(只適用於品牌專員)或ARO合約中指定的信用卡帳戶或以其他方式，收回我/我們按照ARO合約而獲得的所有產品折扣優惠。

I/We understand and agree that in the event that I/we terminate the ARO Agreement before its expiry, Nu Skin may recoup all amounts of product discount I/we received under the ARO Agreement by debiting my/our brand affiliate account (applicable to Brand Affiliates only) or designated credit card in the ARO Agreement or otherwise.
