    Please read the following instructions carefully:
    • Please fill out the form with the required information by typing into the empty fields.
    • Please make sure to use the email address that is registered under your account, otherwise we cannot process this request.
    • Please check if all provided data is correct before submitting your request.
    • By pressing the 'submit' button, we will automatically receive your completed request form. You will receive an automatic confirmation email to acknowledge that the form has been submitted.
    • Once your request is processed, our Market Scanner Cordinator will get back to you via email at your registered email address.

    Return Scanner / Terminate Lease
    • Please download this form by clicking the down arrow icon on the document.
    • Please print the form, fill it out, and upload it to the field below.
    • Please make sure to return the Scanner ensuring that it arrives to the below address by the 15th day of the given month. Otherwise, you will be charged with the next Scanner Monthly Fee.
    • Please make sure to return the Scanner in the original white box it came in or ensure completely secure packaging.
    • Please make sure that the S3 Scanner charger does not touch the Scanner in the package as it might cause damage to the Scanner shell that would have to be deducted from your down payment.

    • PDF Format, File size should be a minimum of 200 Kbytes.
    • JPEG Format, Resolution should be a minimum of 1200x1700 pixels.
    • Additionally, please ensure that the scanned or photographed image is sharp and legible, even when zoomed in.
